If a subscription you will be charged on same day every month until completed. If you are purchasing tests or doing a pre sentence invesigation report you will pay once and done. Card used to set up the payments will be the same card that will be charged every month. At the completion of your program it is your responsability to check in with our Staff making sure the subscription payments has been disabled so card will not be charge. If so and you did not follow through the funds will not be returned. All sales are final. Please let us know if you have any questions.

Nuestro programa de monitoreo electrónico es ahora un programa basado en aplicaciones. Eso es correcto sin dispositivos desgastados. El personal principal le enviará dos aplicaciones y los códigos de la aplicación. Important Information - In Home Breathalyzer Device System

Core Probation in-home breathalyzer program. Clients will be given a breathalyzer or obtain one via USPS. You will have to blow into the devices during the time windows (located below). Client(s) must blow into the device every day during the scheduled windows. Random testing may also be done by one our staff members notifying you the client via a text message, phone call, and push notification. If the test is not taken within 30 minutes of being prompted it will be considered a missed test and is considered a Violation. Violation charges are $100 per violation and needs to be paid in full in order to remain on our program.

Violations consist of any or all of the following: a positive blow, a missed test (even a random test that has not been taken within the time period allotted), and or not following the guidelines put into place by Core Probation and Consulting staff. Violation fees and added conditions may be assessed. Fees assessed is $100 for each violation. Client may also be asked to do a random test ($30). The test will either by Urine or Oral test. Any client going over twelve hours without a test will be called into do a alcohol test. The test is to obtain proof that alcohol was not consumed.

If a positive blow was detected, it is your responsibility to test up to three consecutive tests waiting a few miniutes between each of the additional tests. Not following the outlined rules will also be considered a violation. If the breathalyzer becomes broken in your care. The cost for the repair and the time to replace the device will be assessed $300.

Payment for the following month is due every month on or before the date the device was activated. i.e. If I received the device on the 7th then before or on the 7th the payment is due. If no payment has been rendered the device will be disconnected, you will be removed from the app, and a violation will be sent to the court. It is your responsibility to follow all of these rules and conditions. All terms and conditions are subjected to change. 1st test: 5 - 8 AM 2nd test: noon - 2 pm 3rd test: 5 - 8 PM 4th test: 10 - midnight

Signing service agreement:

I agree to all of terms and conditions outlined above. You will all have access to your case link where you can report in at least once a month to update Core Staff on any changes. The program and services are located on our Core Client App. Program is located in both your caselink and in the Core App. These individual program website will teach you how to use Core program and service(s).

Any violations or disregard to the court orders may result in a contempt charge from the court and/or you may be required to serve jail time. When you have completed the electronic monitoring program you need to notify a member of our Core staff before the apps are deleted from your phone or before your location is turned off. Any questions to the program must be addressed by one of our professional Core Staff.

Core Probation & Consulting can collected and maintain data for both work and research purposes (Ph.D. Dissertation). All videos could be used for training purposes. I agree to pay all costs associated with my individual services including any and all violation fees. Your terms / conditions may change depending on the Judge and or the Court Order. All sales are final with no refunds.

Once you submit the service agreement, please pay attention to the text and email sent to you. Additional links and the monthly invoices will be sent. In your program site you will learn about Core Probation program and/or services. How you are to to check in monthly. Terms and conditions may change. Release of Information to

Core Probation & Consulting

Core Probation & Consulting service agreement form covers a release of information. To share confidential information with the Criminal Justice System (CJS). Educational Studies (Walden University), mental health agencies, and other agencies that Core Probation & Consulting to disccuse your case.

I give permission for, Core Probation & Consulting staff and or other providers who enter into the service agreement can and will be used. I understand the data derived from program may be used for research purposes. Anonymity will be maintained in accordance with Federal, State and Professional Research Laws and Standards.

Education Stipulation

* For any and all fees will be required to paid when signing up for these education program. I will then received my workbook and educational link in my email. If by the third attempt you do not pass you will be requried to pay an additional $50 and a member of the Core Probation & Consulting Staff will use the communication group to assist you through the educational course.

More Information

Fees not paid could end with our Core Staff rejecting the work with notes. No completion certificates of completion will be sent to the court. Court will also see our notes as to why a certificate was rejected. If the balance is not paid in fulland you have completed yourcourse then you can make up the payment to Core and Core will send over the completion certificates the courts.

I undersand, in order to complete the programs and obtain the completion certificate and or doing a motion to terminate probation it is required.I understand I will completed an exit interview with a member of our Core Staff. Late fees will be charged $25 per month as long as the account is in past due status. Credit card on file will be used for charging all fees, violation, and late charges. All sales are final and no refunds are given.
